VON Durham is a non-profit agency that offers community health programs to the residents of Durham

What We Do

VON Durham offers the following programs and services:

ASSISTED LIVING SERVICES FOR HIGH RISK SENIORS – allows individuals to remain in their own homes with personal support assistance, homemaking services, reassurance checks and care coordination.

BEREAVEMENT SERVICES – a safe space for individuals to share their thoughts, feelings, memories, and dreams through our one on one support or in a group setting i.e., spousal, parental or sibling loss, children’s or teens support groups.

BLOOD PRESSURE CHECKS – provided by specially trained volunteers.

CAREGIVER SUPPORT GROUPS – caregivers explore their concerns and feelings in a safe and caring environment.

DAY HOSPICE – offering social interaction, peer support, companionship and information for caregivers and clients in a safe space.

INFORMATION AND EDUCATION – available to individuals and the community at large - these services can include in-house events, and educational opportunities in the community.

PALLIATIVE CARE COMMUNITY TEAM (PCCT) –a multidisciplinary team that provides holistic person centred palliative care to individuals from early diagnosis through bereavement. This care includes: disease management, psychological, social, spiritual, practical, end of life care/death managements, and loss/ grief support.

PALLIATIVE CARE SERVICES – information, referral and advocacy to assist individuals living with a life-threatening illness to stay at home for as long as they may choose. One-to-one support provided through trained compassionate volunteers.

PALLIATIVE PAIN AND SYMPTOM MANAGEMENT CONSULTANT – consultation, education, mentorship and linkages to palliative care resources across the continuum of care throughout Durham Region.

SELECT PROGRAM – specialized nursing supports for the developmental services sector.

SMART EXERCISE – a volunteer-led exercise program designed for 55+ to improve strength, balance and flexibility.

WIN - MENTAL HEALTH CASE MANAGEMENT – nursing support to individuals with chronic mental illness.
